How to Fill Form
- One Time Registration (OTR) is mandatory.

- OBC Certificate must be issued after 01 April 2023.
- Keep all documents ready – Eligibility proof, ID, Photo, Signature, etc.
- Check all columns carefully before final submission.
- Take a printout of the final submitted form.
